Service Description
Community Health Workers (CHWs) play a critical role in supporting patients with complex social and behavioral health needs . Given that CHWs are especially skilled at building trust with hard to reach or historically underserved communities, they may be more likely to encounter patients that present with high or rising risk psychological and behavioral health concerns like traumatic responses and suicidal ideation . This training is designed to provide a CHW audience with Safety Planning strategies and tools to enhance patient safety and to ensure appropriate linkage to essential community programs . This training will also provide a CHW audience with information on how to effectively document these types of patient encounters to ensure continuity of care for their patients .
