Service Description

Presbyterian Community Health offers a Blood Pressure Self-Monitoring Program to help you learn how to lower your blood pressure. The program will teach you how to eat to lower blood pressure and monitor it at home. You will also receive one-on-one support from our Healthy Heart Ambassador.
